Without internal controller. With two SC/PC connectors. With 16-pin electrical interface. 
Sorry, we cannot find datasheet, but we will print a copy of datasheet and Technical Note of similar model: 17dBm JDS MAC1-1 EDFA module, the pinout info should be same as JDS MAC1-1 EDFA module. The last two photos are for similar model MAC1-1 only.
